Scheduling your initial assessment.  

Please click the button below to schedule your initial assessment. Our billing company Headway will verify your insurance and help you set up your payment method if you are not using insurance. After your insurance/payment method has been verified, you will receive an invite to the patient portal. There you will find new patient intake paperwork to complete so that we can better understand your needs. 

Please complete all paperwork 24 hours prior to your appointment.
